About This Procedure

Lower eyelid surgery is planned around a structural question: where exactly does the eye area change, and what is causing it there.

Lower lid concerns are usually some combination of three things. Fat sitting forward of the bone that forms the lower edge of the eye socket produces a bulge. A hollow immediately below that bulge, where the lid meets the cheek, produces the shadow patients describe as looking tired. And skin that has lost elasticity produces fine crepe-like texture that is a separate problem from either.

Each responds to something different. Addressing the bulge alone, in a patient whose main concern is the shadow, produces a flatter but still tired-looking eye. Addressing the texture alone, where the contour is the issue, produces smoother skin over an unchanged shape. The assessment at consultation establishes which of the three is driving the appearance, and the surgical approach follows from that.

How the incision is placed also varies. Where skin removal is not required, the work can be done from inside the lid. Where it is, a fine incision beneath the lash line is used.
